MAKING AN APPOINTMENT?

Prior to seeing your animal, I must have received back the veterinary consent form and Pup profile. 

I work under the Veterinary Surgeons Act 1966 and Exemptions Order 2015 and therefore will not treat your dog without prior veterinary approval. 

Please send the completed forms via email prior to your appointment. Unfortunately, if this is not obtained the appointment will need to be rescheduled.
